    1. Introduction

    This Payment Policy outlines the terms and conditions governing the payment process between Healthcare Intermediaries LLC (“the Company”) and its patients (“the Patients”) for the telemedicine services provided by the affiliated physicians. Healthcare Intermediaries LLC is a facilitator of patient-provider relationships and is not a medical provider.

    2. Payment Methods and Platforms

    Patients are required to make payments directly to Healthcare Intermediaries LLC through the following payment platforms: authorize.net, PaymentCloud, and JotForm. The Company does not accept payments in any other form or through any other platforms.

    3. Subscription Plans and Recurring Payments

    Healthcare Intermediaries LLC offers subscription plans that run on a recurring basis, typically monthly or quarterly. By signing up for a subscription plan, Patients agree to be enrolled in an automatically renewing subscription. The Patients’ credit card or preferred payment method will be charged automatically at the beginning of each billing cycle (monthly or quarterly) until the subscription is canceled.

    4. Cancellation Policy

    To cancel a subscription, Patients must send an email to [email protected]. The cancellation request must include the Patient’s full name, email address, and subscription plan details. The cancellation will take effect from the next billing cycle, and no further charges will be applied after the cancellation is confirmed. No refunds will be issued for the current billing cycle.

    5. Declined Payments and Outstanding Balances

    If a Patient’s credit card or preferred payment method is declined for any reason, the Patient remains responsible for the outstanding balance until they notify Healthcare Intermediaries LLC in writing of their cancellation. The Company reserves the right to attempt charging the Patient’s card or payment method until the outstanding balance is settled or the subscription is canceled.

    6. Late Payments and Penalties

    If a Patient fails to make a payment on time, Healthcare Intermediaries LLC may charge a late payment fee. The Company also reserves the right to suspend or terminate the Patient’s access to telemedicine services until the outstanding balance is settled.

    7. Changes to Payment Policy

    Healthcare Intermediaries LLC reserves the right to modify this Payment Policy at any time. Any changes will be effective immediately upon posting the revised policy on the Company’s website. It is the Patient’s responsibility to review the Payment Policy periodically for any updates.

    8. Refund Policy

    Healthcare Intermediaries LLC does not offer refunds for any completed payments or partially used billing cycles. If a Patient believes they have been charged in error, they must contact the Company at [email protected] within 30 days of the charge in question. The Company will investigate the claim and issue a refund if deemed appropriate.

    9. Payment Disputes

If the dispute cannot be resolved through direct communication, Patients may initiate a chargeback or dispute process through their credit card issuer or payment platform. The Company uses secure payment platforms (authorize.net, PaymentCloud, and JotForm) to process payments and ensure the confidentiality of Patients’ financial data. However, the Company cannot guarantee the absolute security of payment information transmitted over the internet. Patients are responsible for safeguarding their payment information and notifying the Company immediately if they suspect any unauthorized use of their payment details.

    11. Payment Confirmation

    Upon successful completion of a payment, Patients will receive a payment confirmation email from Healthcare Intermediaries LLC. This email will include details of the subscription plan, payment amount, and billing cycle. Patients are advised to keep a copy of this email for their records.

    12. Payment Information Updates

    It is the Patient’s responsibility to ensure that their payment information, including credit card details and billing address, is up-to-date and accurate. Failure to maintain accurate payment information may result in the suspension or termination of telemedicine services.

    13. Currency and Taxes

    14. Governing Law

By making a payment to the Company, Patients agree to submit to the exclusive jurisdiction of the courts located within that state for the resolution of any disputes.

    No waiver of any term or condition of this Payment Policy shall be deemed a further or continuing waiver of such term or condition or any other term or condition, and Healthcare Intermediaries LLC’s failure to assert any right or provision under this policy shall not constitute a waiver of such right or provision.

    20. Entire Agreement

    This Payment Policy constitutes the entire agreement between Healthcare Intermediaries LLC and the Patients with respect to the subject matter hereof and supersedes all prior or contemporaneous understandings, agreements, negotiations, representations, and warranties, both written and oral, with respect to such subject matter.
